My general feeling is that the citrus allergy is likely real, if only because it limits the foods Rodney can eat, and I don't see him doing that for something that's not real.
The hypoglycemia, eh, I suspect he's a lot like me and gets icky when his blood sugar drops. I don't have any problems with fiction that treats it as real, but my general sense is that it's probably an exaggeration.
I don't actually see them treated all that derisively on the show, any more than Rodeny's tendency to talk about how brilliant he is.
My one friend who is really hypoglycemic, by the way, is INSANELY boring about it. If we had the kind of friendship where we poked fun at one another regularly, I would make all the fun in the world of her hypoglycemia -- but that doesn't mean it isn't real, or something I've helped her deal a couple of times as necessary.
And I think when it's presented as funny on the show, it's mostly because Rodney invokes it in excessive and/or inappropriate ways. I.e., "we're serving lemon chicken." "Does that have lemon in it?" It's all part of Rodney's colossal self-absorption.
